Forms and Accommodation Applications

Application for Accommodations & Services
Please allow at least one week for the Office of Accessibility(O.A.) to review each application and supporting documentation.

Accommodation Letter Request Form
Students registered with the O.A notify their professors of their disability related accommodation needs when they present them with Accommodations Letters signed by the director for students with disabilities. Students with disabilities are advised to request and present these letters during the first week of each new semester so that accommodations can be provided in a timely manner.

Request for Assistive Technology in the Classroom/ Labs
Requests for assistive technology must be made at least three weeks in advance. Students are advised to submit all requests for AT in their classrooms at the time of registration.

Request for Books-on-Tape
Students requesting books-on-tape must submit a list of classes, professors, and all required textbooks to the Office of Accessibility(O.A.). Students are advised to submit this list prior to start of the semester. Books-on-Tape can take up several weeks to receive. Books-on-tape are available on loan only; for one semester. Students must return books-on-tape at the end of each semester.

Request for Sign-Language Interpreting Services
Three days notice is required for requests for sign-language interpreting services. Students needing sign-language interpreting services must make requests for these services to Office of Accessibility(O.A.), not the individual interpreters or agencies.
