Student Document Check List

Prospective students must submit proof of income, education, citizenship and residency and provide one form of documentation from each category:

I. Proof of Legal Status in the United States
____ U.S. Birth Certificate
____ U.S. Citizenship/Naturalization Certificate
____ U.S. Passport
____ U.S. Permanent Resident card
____ U.S. Asylum documentation


II. Proof of New York State Residency
____ NYS Driver’s License or NYS DMV Identification (must be dated at least one year prior to registration)
____ NYS voter registration card
____ Apartment Lease
____ Residential (Shelter) Program letter
____ Posted mail from a bank with name and address (must be dated at least one year prior to registration)
____ New York State income tax return (IT 201, IT 150 and IT2 W-2 summary)
   
III. Proof of Education
____ High School Diploma or Transcript (with graduation date and school seal)
____ Foreign Diploma (must be translated into English)
____ Student Dismissal letter
____ High School Graduation Verification letter
____ High School Equivalency Diploma (including test scores)

      (Effective January 2, 2014 our GED program name changed to High School Equivalency (HSE))
____ College Transcript

(Applicants with 24 or more college credits are not eligible.)
   
IV. Proof of Income - (All that apply)

      (Pay stubs, W2 forms or notarized letters are not acceptable as proof of income.)
____ Federal Tax Return (1040, 1040A, or 1040EZ)
____ Non-filers: IRS Form 4506-T (provided by Admissions Office)
____ HRA Current budget letter (most recent & not older than 90 days)
____ SSA/SSI Documentation – Social Security Award letter for current year
____ Veterans Administration Disability Benefits letter
____ Unemployment, Pension, Annuity Benefits or Workers Compensation letter for current year
____ Child Support or Alimony payment stubs for current year. (Court ordered or affidavit)
____ Residential (Shelter) Program Letter
   
V. Photo ID
____ U.S. Passport
____ Permanent Resident Card
____ New York State Photo ID
____ Benefit Card
